If you are a patient we will collect personal information, including (but not limited to), your name, date of birth, contact information, health insurance details and relevant sensitive health information. This is so we are able to provide you with the best possible dental care.
If you are a contractor, service provider, supplier or potential job applicant, we may collect personal information such as your name, contact information and other information we require to do business with you, including by not limited to your professional qualifications, registrations, banking information, etc.
If you are a patient, we will collect your personal information, including sensitive health information directly from you, unless you provide us with the authority to obtain this information from other sources such as a family member, carer or from another healthcare professional.
If you are a contractor, service provider, supplier or potential job applicant, we will ordinarily collect personal information directly from you. However, in some circumstances, we may conduct searches that are publicly available information and/or conduct background checks (we will advise you if this is required).
If you are a patient, we may disclose your personal information to other healthcare practitioners that are actively involved in your care, your health insurer and/or any other health professional who requires your information.
